AKC/Eukanuba National Championship Eye Clinic Information

Event: OFA Eye Certification Exams Eye Clinic by Melanie Church, DVM, DACVO – Board Certificed Veterinary Ophthalmologist

Date: Friday, December 11th & Saturday, December 12th

Time: Friday – 1:30 PM – 6 PM (or as needed) Saturday – 9 AM – 5 PM (or as needed)

Location: Near Show Veterinarian

The price is $40 per dog by cash only. Includes eye dilation, eye exam and completed OFA eye certification form. Please bring dog’s registration name, registration number, owner’s name and contact information, date of birth, sex, breed and permanent identification information (microchip/tattoo).

To pre-register email dr.fauna@gmail.com with dog’s information and desired time and date for the eye exam. All others will be worked into the schedule on the days of the event.

The OFA eye certification exam does not include any diagnostics and is not intended as a consultation for eye problems. The completed form is to be mailed and certificates are issued at the discretion of the OFA organization.

Note: Dogs eyes will be dilated and then the exam will be performed 10-15 minutes later. This should be kept in mind when signing up, as it is recommended that the exam is given 4-6 hours before showing or after showing.
